If you would like to make a request to amend your medical records, please complete the Request for Amendment of Health Information Form and return it to the Health Information Management department at National Jewish Health.
Download, complete, and print the Request for Amendment of Health Information Form, making sure to include your handwritten signature.

Medical Record Destruction Notices
On April 28, 2026, National Jewish Health will destroy all archived medical records of persons, Adults exceeding 10 years who received treatment prior to 02/28/2016 at National Jewish Health’s South Denver Clinic, which was located at 499 East Hampden Avenue, Englewood, CO 80113, Suite No. 300. Please direct inquiries concerning records to the Health Information Management Department (303)398-1580 between 8:00am and 4:30pm, Monday-Friday. Inquiries for the entire paper record will not be accepted after April 27th, 2026.
