Agreement to Terms and Conditions

By accessing and using the njhealth.org website and/or the National Jewish Health patient portal (Service) you accept the contents of this Agreement. If you do not agree to the Terms and Conditions for this Service, you may not use My National Jewish Health patient portal.

We may modify this Agreement at anytime. Any modifications made to this Agreement will be effective immediately upon posting on the Service. This Agreement may be viewed at any time while using the Service by clicking the "Policies & Guidelines" link at the bottom of each web page. By accessing or using My National Jewish Health, you agree to be bound by the Terms and Conditions posted on the Service at the time of your access or use. You agree to review the Terms and Conditions posted on My National Jewish Health each time you use My National Jewish Health so that you are aware of any modifications made to this Agreement.

We reserve the right at any time and from time to time to modify or discontinue, temporarily or permanently, My National Jewish Health, or any part thereof, with or without notice.

Our failure to act with respect to a breach by you or others of these Terms and Conditions does not waive our right to act with respect to subsequent or similar breaches. These Terms and Conditions set forth the entire understanding and agreement between us and any Service User with respect to the Service, its uses and services.

 

Emergency Treatment

My National Jewish Health should not be used for emergency or urgent medical issues that require immediate medical attention. In a medical emergency, dial 911 (in the United States) or call or seek care with your local emergency services provider or Emergency Room.

Purpose

National Jewish Health provides a main website: www.njhealth.org (Website). This website allows our patients and other users to view health-related information and access My National Jewish Health patient portal in order to communicate with our practitioners, our other staff, and arrange for clinical services and access additional services.

The general information provided on the Website or My National Jewish Health is not a substitute for the advice of your personal physician or other qualified health care professional. Always seek the advice of your physician or other qualified health care professional with any questions you may have regarding your medical symptoms or a medical condition. Never disregard professional medical advice or delay in seeking it because of something you have read on the Website.

From the Website a patient or user can set up and access his or her My National Jewish Health account via the patient portal. The patient portal provides the user a secure and confidential way to request appointments, communicate with his or her physician or heath care team, and to access a limited number of test or laboratory results.

 

User Participation in Interactive Services

Any personal information you submit to the Website on My National Jewish Health is governed by our Privacy Statement (A link to our privacy practices is available at the bottom of each Website under Policies and Procedures). This includes information on your rights to see and receive copies of your personal health information. To deactivate My National Jewish Health online account, please contact our My National Jewish Help Desk at 303.398.1666 during business hours or ISCentral@NJHealth.org 24 hours per day. To the extent there is an inconsistency between these Terms and Conditions and the Website Privacy Statement, these Terms and Conditions shall govern.

Members may use My National Jewish Health to view certain information displayed from their medical records, including the results of certain laboratory tests. Your use of the lab test result feature is considered to be a request by you to receive your lab test results online or on your mobile device.

In addition, although My National Jewish Health displays certain information from your medical record, it does not necessarily display all information found in your medical record. If you think that your medical record information displayed on My National Jewish Health is inaccurate, you can request National Jewish Health to amend your medical record by mailing a request to the department identified in our Notice of Privacy Practices. To request a complete copy of your medical record, please contact our Health Information Management department on our main campus.

At the age of 18 an individual has the right to consent to treatment. At the age of 18 a child’s access as well as his or her parent’s access to their account will be suspended. The child will then have full rights to re-activate their My National Jewish Account with their own sign-on credentials. To reactivate their account the patient will need to call the My National Jewish Help Desk at 303.398.1666

National Jewish Health provides you with a number of interactive online services to help you better manage your health. These services may include (but are not limited to):

  • secure messaging, including:
    • email your doctor's office
    • appointment requests
  • viewing limited types of medical or lab results
  • viewing medical record requests
  • requesting prescription refill

You agree that you will not upload or transmit any communications or content of any type (including secure messaging) that infringe upon, misappropriate or violate any rights of any party.

In consideration of being allowed to use My National Jewish Health’s interactive services, you agree that the following actions shall constitute a material breach of these Terms and Conditions:

  • signing on as another person without the permission of the authorized patient/user
  • using secure messaging for any purpose in violation of local, state, national, international laws or posted National Jewish Health policies
  • transmitting material that infringes or violates the intellectual property rights of others or the privacy or publicity rights of others
  • transmitting material that is unlawful, obscene, defamatory, predatory of minors, threatening, harassing, abusive, slanderous, or hateful to any person (including National Jewish Health personnel) or entity as determined by National Jewish Health in its sole discretion
  • using interactive services in a way that is intended to harm, or a reasonable person would understand would likely result in harm, to the user or others
  • collecting information about others, including email addresses
  • intentionally distributing viruses or other harmful computer code

National Jewish Health expressly reserves the right, in its sole discretion, to terminate a user's access to any interactive services and/or to any or all other areas of the Websites due to any act that would constitute a violation of these Terms and Conditions.

 

Links

This National Jewish Health website provides links to other websites that are not owned or controlled by National Jewish Health ("Third Party Websites").

  • National Jewish Health provides links to Third Party Websites to connect you easily to additional sources of health information or third party services that may be of interest to you. We may not have any business relationship with the party that controls this type of Third Party website and a link to such a site is offered only as a convenience to you.
  • National Jewish Health also provides links to Third Party Websites managed by vendors that we have made arrangements with to offer you services to help you manage your health or to take and fulfill orders when you purchase items or materials from us. These Third Party Websites may be co-branded, meaning that they display the National Jewish Health logo and the logo of the Third Party vendor but they are owned and controlled by the Third Party.

When practicable, we will let you know when you are leaving the Website and linking to a Third Party website. For example, we may include a notice advising you that you are being sent to a Third Party Website on our web page adjacent to a link or on the first page that appears after clicking on a link.

National Jewish Health is not responsible for the content, security or the privacy practices of Third Party Websites. Please review the privacy statement and any terms of use of each Third Party website you visit. Unless we specifically advise you otherwise, links to Third Party Websites do not constitute or imply endorsement by National Jewish Health of those sites, the information they contain or any products or services they describe. National Jewish Health does not receive payment or other remuneration in exchange for you linking to or using any Third Party Website.

 

Access, Correction, and Data Integrity

Although we attempt to maintain the integrity and accuracy of the information on the Website, we make no guarantees as to its correctness, completeness, or accuracy. The Website may contain typographical errors, inaccuracies, or other errors or omissions. Also, unauthorized additions, deletions, or alterations could be made to the Websites by third parties without our knowledge. If you believe that information found on the Websites is inaccurate or unauthorized, please inform us by contacting our Web Manager. Use by Children

We do not knowingly allow National Jewish Health patients under the age of 18 to create accounts that allow access to the secured features of the patient portal.

 

Email Communications with National Jewish Health Patients

By accepting these Terms and Conditions, you agree to receive essential communications by email. These communications may include (but are not limited to):

  • notification that an important message awaits you on the Website (this may be a message from your doctor, a new lab test result, an appointment confirmation, a response from a pharmacist, etc.)
  • emergency alerts and critical messages (for example, if a snowstorm or wildfire has temporarily closed a medical center)
  • general health and news communications from National Jewish Health

As part of your essential communications and services, you will receive update reminders by email. In addition, you may periodically receive online invitations to participate in member satisfaction and other types of surveys related to your experiences at National Jewish Health. Your participation in such surveys is always optional.

 

Passwords

National Jewish Health has several tools that allow you to record and store information. You are responsible for taking all reasonable steps to ensure that no unauthorized person shall have access to your My National Jewish Health online password or account. It is your sole responsibility to (1) control the disclosure and use of your activation codes and password; (2) authorize, monitor, and control access to and use of your National Jewish Health online account; and (3) promptly inform the My National Jewish Help Desk at ISCentral@NJHealth.org or call 303.398.1666 of any need to deactivate a password.

 

Portal Notifications

Within My National Jewish Health users may designate what communications they wish to receive and those they would like to opt-out of receiving. Users may click on the Notification Preferences Tab they will be able make their selections

 

Security

The security of any data you submit via this site is very important to us. This site uses encryption technology, and we constantly review our security practices to help ensure that your data is maintained securely. However, no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information sent to us. Moreover, when you transmit information via the internet, your information will be transmitted over a medium that is beyond our control, and therefore the security of the transmission may be compromised before it reaches us. Accordingly, National Jewish Health makes no guarantee as to confidentiality or security. IN Addition, YOU HEREBY EXPRESSLY AND SOLELY ASSUME THE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH YOUR USE OF YOUR ONLINE SERVICES ACCOUNT.

 

Termination of Service

My National Jewish Health account and any application or website accessed via this account, is being provided by National Jewish Health as a courtesy to you, and National Jewish Health reserves the right to terminate modify, discontinue, temporarily or permanently, the online services account and any application or website accessed via this account, at any time, with or without notice. In addition, National Jewish Health may terminate, modify or limit your online services account or access to any website or application based on your online services account if you violate the terms of this Agreement or any other agreement with National Jewish Health, you violate any law, or if we determine, for any other reason, that it is no longer appropriate for you to use the online services account or access certain services using your online services account.

The National Jewish Health website is designed for educational purposes only. You should not rely on this information as a substitute for personal medical attention, diagnosis or hands-on treatment. Except as otherwise indicated, the My National Jewish Health patient portal and all content on the Website, including text, graphics, logos, button icons, photos, images, forms, audio, video, questionnaires, and software, is the property of National Jewish Health or its licensors and is protected by United States and international copyright laws. National Jewish Health allows you to view or download a single copy of the material on the website solely for your personal and noncommercial use.

The compilation of all content on the Website is the exclusive property of National Jewish Health and is protected by United States and international copyright laws. Unless specifically authorized in writing by National Jewish Health, any use of these materials, or of any materials contributed to the Website by entities other than National Jewish Health, on any other website or networked computer environment for any purpose is prohibited. Users of the Website are not authorized to make any use of the National Jewish Health marks or the proprietary marks of third parties, including but not limited to, as metatags or in any other fashion that may create a false or misleading impression of affiliation or sponsorship with or by National Jewish Health or the applicable third party.
