The 4 Levels of Zero Suicide and NJH

Zero Suicide Level 1 Leadership and Planning

  1. Leadership Buy-In

    • Received written commitment from Dr Horn

  2. Implementation Team

  3. Organizational Self Study

    • NJH Council Members attend monthly Zero Suicide Learning Collaborative hosted by the Office of Suicide Prevention

    • Completed Self-Survey in December 2021

    • Completed the Workforce Survey in November 2021

    • Ligature Risk Assessments are completed on a yearly and as needed basis

  4. Work Plan

    • Council will complete the Zero Suicide Work Plan in 2022

      • Plan includes SWOT analysis and environmental scans

 

Zero Suicide Level 2 Training

  1. Workforce Survey

    • Completed the Workforce Survey in November 2021

  2. Non-Clinical Workforce Training

    • Suicide Awareness and Response Training

    • NJH Suicide Awareness Quarterly Virtual Learning Collaborative

  3. Clinical Workforce Training

    • Suicide Awareness is part of Skills Review for Nursing

Zero Suicide Level 3 Identify, Treat, Engage

  1. Screening

    • Currently, tracking the number of patients Social Work is notified of who identify at risk in outpatient settings

    • All Inpatients are tracked 

  2. Assessment

  3. Safety Planning

    • Will be evaluating the policy in 2022 to make sure all individuals who screen positive for suicide risk have a safety plan initiated.

  4.  Engagement for Health Equity

    • Will implement in 2022 soliciting input on suicide-related policies and procedures from organizations in the community that serve people who are affected by health inequities.

 

Zero Suicide Level 4 Transition and Improve

  1. Follow-Up with-in 3 calendar days.

    • Not a current policy within NJH

  2. Data Collection & Tracking

    • Currently tracking Inpatient at 100%

    • Tracking outpatient Suicide Awareness Council is informed of
