Verify NJH Affiliation 

Please use this site to verify NJH affiliation and privileges. Both documents are generated at the start of each month, and contain current information on the day you print the verification.

Instructions:

  1. Click the link below according to the provider’s status with NJH.

  2. When the PDF document opens, hold down the Ctrl key, and press the “F” key.

  3. In the search box, type the provider’s last name, and press the enter key.

  4. Print the page(s) for the provider. You can print to paper, or to a separate PDF that you can save in your files.
