What we do? At National Jewish Health, we help patients with diabetes and pre-diabetes to achieve their best possible health status and quality of life, by providing outstanding patient-focused education, skills training, and support.
The National Jewish Health Diabetes Self-Management Education (DSME) Program is committed to providing the most advanced care for patients with pre-diabetes, and type-2 diabetes. Our care team includes specialty-trained nephrology (kidney) and diabetology (diabetes) doctors, nurses and dietitians.
The DSME Program is appropriate for all patients with pre-diabetes, type 1 and type 2 diabetes, and can help with:
- Understanding a recent diabetes diagnosis
- Learning or re-learning self-management skills
- Improving glucose pattern management
- Targeting nutritional needs
The program uses principles of the American Association of Diabetes Educators’ (AADE) 7 self-care behavior framework. An individual assessment and prioritized plan is developed with ongoing self-management support offered.
Why National Jewish Health? Our institution strives for excellent medical care. Many of our patients have chronic disease and are at risk for developing diabetes or have been diagnosed with type 2 diabetes. We created a comprehensive diabetes management program with national experts as a convenience to current patients and to help others who are new to National Jewish Health manage their disease.
Nationally Accredited Program
The National Jewish Health Diabetes Self-Management Education Program was named an accredited diabetes education program by the American Association of Diabetes Educators (AADE) in June 2013. This will allow residents in and around the Rocky Mountains increased access to critical diabetes education services.
“Trends show that diabetes education is moving out of the hospital and into the community, so AADE’s accreditation program was created, in part, to encourage diabetes education where the patient is seeking care,” said Leslie E. Kolb, RN, BSN, MBA, Program Director, AADE Diabetes Education Accreditation Program. “The National Jewish Health Diabetes Self-Management Education Program is exactly the type of program we envisioned when we set up our accreditation program in 2009.”
