Submitting a Sample

To submit a sample for testing:

  1. Choose the appropriate test from the list of available tests below.

  2. Download and fill out a Mycobacteriology requisition.

  3. Package the sample for shipping following the collection/transport instructions for the test on our website.

  4. Send the requisition and sample to the address in the upper right corner of the requisition.

  5. To request test pricing and payment information, contact our Client Services Team at 800.550.6227.
