Final Checklist before Submitting an Application

  1. Provisions have been made to minimize risks and those procedures are documented in application.
  2. Provisions have been made and documented to care for subjects in case of accident or injury.
  3. Provisions have been made to obtain informed consent from all individuals related to the study. (e.g. parents, subjects, etc.)
  4. All questions on the application have been completed.
  5. All supporting documents have been attached. This includes surveys, interview questions, solicitation letters, flyers, advertisements, etc.
  6. Appendixes are completed and signed by the Principle Investigator.
  7. For Full IRB Review, please attach 22 copies of the application and consent form(s), and appendixes. For expedited review, please attach 2 copies of the application, consent form(s), and appendixes. (Failure to include the necessary number of copies of all information will delay the evaluation process until the investigator forwards additional copies.)
