Use the attached Requisition Form in PDF format to request samples. Make as many photocopies as you need.
All specimens sent to the laboratory should conform to all Federal and IATA shipping regulations.

Sample:
Serum, ambient temperature:
- Draw enough blood in a red-top tube to provide the required amount of serum specified for each test (yield is approximately 1 ml serum per 2.5 ml of blood). Allow the blood to clot and then centrifuge within one hour.
- Transfer the serum, free of blood cells, to a clean shipping vial. Place the specimen vial(s) in a Styrofoam container, and tape closed.
EDTA plasma, ambient temperature:
- Spin down purple-top tube as soon as possible and transfer the plasma to the clean shipping vial.
- Ship as described above.
Heparinized whole blood, ambient temperature:
- Place green-top tube with whole blood into a Styrofoam container, and tape closed.
- Mail the container and requisition form together.
Serum, frozen:
- If you need to ship serum or other samples that require dry ice (such as complement specimen or other labile components), please put the specimen tubes into a Ziploc or other sealed plastic bag.
- Place the Ziploc bag into a Styrofoam container with enough dry ice to last for 48 hours (2 to 5 lbs).
- Place the Styrofoam container into a cardboard container.
- Ship by overnight delivery to arrive at National Jewish Health on a weekday. Do not ship packages on Thursday or Friday except by special arrangement with the laboratory.
EDTA plasma frozen:
- Plasma or other fluids for complement studies or other tests should be sent as described above for frozen serum.
