Data Quality Cycle – QA Report 1

State-Reportable?

Subjects brought in under the patient selection rules will include some that do not need to be reported to the state. We need to resolve quickly whether a tumor record should be started for them. The oncology group marks those that do not need to be state reported in this QA report, changes the EHR to reflect state-reportable for those that should be reported to the state, or notes that further investigation is needed.

This report is currently sent as an Excel spreadsheet by e-mail to the appropriate oncology clinician, filtered so that only their patients are visible. Reports are also sent to the Interventional Pulmonology and GI departments. They note resolutions and comments and return them by e-mail. The resolutions are recorded, including a resolution date and a resolved by user name, in the QAIssue table.

Information Recorded for Resolution of Report 1 Issues

Resolutions are recorded in a QA Issue table within the following fields:

  • Resolution Code (ResolutionCode) – see resolution list above.
  • Resolution Comment (ResolutionDesc) – text notes entered by the person entering the resolution. This field is optional.
  • Resolved On (IssueResolvedDt) – the current date/time when the resolution was recorded by the system.
  • Resolved By (ResolvedByUser) – the name of the person entering the resolution into the system.
  • Completed On (IssueCompletedDt) – the current date/time when the system records the issue as complete
