Disease Management Programs For Participants
Are you eligible to participate in a Respiratory Disease Management program offered by National Jewish in collaboration with your health plan or employer? If so, you have an opportunity to work one-on-one with a respiratory trained nurse to help you manage your, or your child's, asthma. Our nurses also work with individuals who have been diagnosed with Chronic Obstructive Pulmonary Disease (COPD).
Good self-management of these chronic conditions mean less trips to the emergency room or hospital and a better quality of life. National Jewish offers numerous valuable tools to help you understand asthma or COPD and what you can do about it. All information is based on national standards of care established by the National Heart, Lung and Blood Institute (NHLBI) Guidelines.
Program Services for Participants
Participants in the program have access to the following program services:
- A peak flow meter with instructions and benefit of use
- Personalized, ongoing and scheduled telephonic educational sessions with a Care Manager assigned to you
- Periodic mailings sent to your home with important information about your condition, perfect for referring back to anytime you need a reminder
- Web-Based Interactive Diary to track you peak flow readings and identify trends in your breathing
- 24/7 access to a respiratory trained nurse to answer questions or concerns any time of day or night
- An Asthma or COPD Action Plan to tell you what to look for in a worsening of symptoms and what to do about it
- Asthma Wizard interactive, animated information source for children with Asthma
- You can request additional educational materials based on your specific questions or situation

Participants in a National Jewish Asthma or COPD program report an average of 95% satisfaction rate with the program.
